In this activity you add a new page to your existing survey template, and add questions expanding on the customers opinions of service. These questions can include but are not limited to:
How long were you on hold?
Do you want a manager to contact you?
Please add any other general comments?
It is important that your questions try out a variety of answer types.
After your new page is created, go back and add text that welcomes the customers and gives any necessary instructions. Also add images to each page to make a them cohesive.
Finally, add a confirmation page the displays customer information from answers, and gives the customer a ticket number.
Things to Think About
If possible, add all the elements to your service item before starting any styling. It will save you time and headaches.
